Dr Ioannis Spiliotis is a Consultant in Diabetes, Endocrinology and General Medicine at Milton Keynes University Hospital. He qualified in medicine at University College London in 2009, after a first-class degree in molecular biology at King's College London. He completed both his specialty training in diabetes and endocrinology and his doctorate in Oxford, on the academic clinical pathway: first as an Academic Clinical Fellow, then, after an NIHR-funded DPhil in Medical Sciences at the University of Oxford's Centre for Diabetes, Endocrinology and Metabolism (OCDEM), as a Clinical Lecturer. He is an honorary researcher at the University of Oxford.

Clinical practice

What Dr Spiliotis sees

Dr Spiliotis sees a wide range of endocrine and metabolic conditions: thyroid disease, diabetes and diabetes technology (including less common and harder-to-classify forms such as monogenic diabetes, MODY), weight and metabolic health (including weight management in type 1 diabetes), PCOS / PMOS and women's hormone health, pituitary and adrenal disorders, parathyroid and calcium disorders, and men's hormone health.

Within the NHS he leads inpatient diabetes care and the young-adult transition service, co-leads the multidisciplinary diabetic foot clinic, works within the insulin pump team on hybrid closed-loop care, and co-authors hospital-wide clinical guidelines in diabetes and endocrinology.

Research

Research & clinical trials

Dr Spiliotis's research focuses on the hormonal regulation of blood glucose, and in particular on glucagon and the prevention of hypoglycaemia in diabetes. He is Chief Investigator of the LEGEND-D trial, published in eBioMedicine in 2026, and was previously Chief Investigator of the LEGEND-A pilot, the Glucagon in MODY study and the DEPTH exercise study.

He is local Principal Investigator for the ACTIVATE trial (digital health in type 2 diabetes), and site Principal Investigator for VAPOR 1 (a pancreatic cancer biomarker study) and for a clinical evaluation of a continuous glucose monitoring system.

In the laboratory

Research-led care

His laboratory work, in the Rorsman laboratory at OCDEM, studied how the hormones that control blood glucose regulate one another, and what happens when that control fails in diabetes and hypoglycaemia.

The same way of thinking informs how he chooses investigations and reads results.
